There are several heparin class drugs (fractions of unfractionated heparin) with lower molecular weight that have names like enoxaparin (Lovenox)

Heparin Mechanism
Medscape:
Mechanism for low dose: Inactivates factor Xa and inhibits conversion of prothrombin to thrombin
Mechanism for high dose: Inactivates factors IX, X, XI, and XII and thrombin and inhibits conversion of fibrinogen to fibrin
Also inhibits activation of factor VIII

Capital: 4-8 hours
Heparin Indications
Capital:
Acute myocardial infarction
Prophylaxis and treatment of thromboembolic disorders (DVT, PE)
Heparin Contraindications
Capital:
Hypersensitivity
Active bleeding
Recent intracranial, intra-spinal, or eye surgery
Severe hypertension
Heparin Side Effects/ Precautions/ Comments
Capital:
Hemorrhage, Hematuria, GI bleeding, Shock, Pain, Bruising, Thrombocytopenia
Dose not lyse existing clots
Chance of bleeding increases when used in conjunction with other anticoagulants
Heparin Adult Dose
Capital:
Bolus: 60 U/kg, max 4,000 U IV/SQ
Infusion: 12-15 U/kg/hr, max 1,000 U/hr
