Albuterol Sulfate Mechanism

A

Medscape:
Beta2 receptor agonist with some beta1 activity; relaxes bronchial smooth muscle with little effect on heart rate

Capital:
Selectively binds to β-2 adrenergic receptors to cause smooth muscle relaxation in bronchioles, which in turn causes bronchodilation

A

Capital:
Bronchospasm. Can be combined with ipratropium bromide to create a DuoNeb.

CT Protocols:
Respiratory distress with bronchospasm.
Moderate to severe hyperkalemia

Capital:
Headache, Tachycardia, Hypertension, Palpitations, Increased myocardial oxygen demand, Nausea/vomiting, Chest pain, Cough, Tremors, Dry mouth, Anxiety

Synergistic effect with other sympathomimetics
β-blockers may blunt effects

Albuterol Sulfate Adult Dose

A

CT Protocols:
Allergic Reaction/Anaphylaxis
2.5 mg via nebulizer; May repeat 2.5 mg via nebulizer,
repeat every 5 minutes (4 doses total)

Asthma/COPD/RAD
* 4-6 puffs per dose of MDI; May repeat every 5 minutes, as needed.
* Initial treatment should be 2.5 mg albuterol and .5mg ipratropium (DuoNeb); May repeat every 5 minutes (3 doses total).
* Following 3 DuoNeb treatments, 2.5 mg albuterol via nebulizer every 5 minutes, as needed.

Hyperkalemia
2.5 mg via nebulizer. Repeat continuously up to a max total of 20mg.

Capital:
2.5 mg in 2.5 ml NS (total 3 doses) via nebulizer with 6-8 lpm oxygen
