Amiodarone Mechanism

A

Potassium channel blocker that inhibits the efflux of potassium from the cell leading to a prolonged QT interval, prolonging the effective refractory period leading decreased myocyte excitability. Also, blocks beta-1 receptors and calcium and sodium channels leading to
decreased AV nodal conduction and SA node function

A

Capital:
Prolonged QT Interval, Bradycardia, Heart block, Hypotension, CHF/pulmonary edema, Cough, Shortness of breath, Nausea, Dizziness, Burning at IV site.
Long half-life (46-58 days)

Amiodarone Adult Dose

A

CT Protocols:
Cardiac Arrest
V-Fib/Pulseless V-Tach
300 mg IV/IO push. Repeat dose of 150 mg IV/IO push for recurrent episodes.
Tachycardia
Wide complex tachycardia
150 mg over 10 min. May repeat once in 10 minutes.
If successful, consider maintenance infusion of 1 mg/minute.
