rheumatologic diseases
–monoarticular:Trauma,gout,osteoarthritis , septic arthritis , lyme disease
–oligoarthritis :such as osteoarthritis also ,touch 2 to 4 joints
–polyarthritis :more than 5 joints ;RHEUMATOID ARTHRITIS ,SLE
to be clear for Rheumatoid arthritis
-it can start as monoarthritis ,oligoarthritis , or polyarthritis ;but the advanced disease is almost always polyarthritis
rheumatoid arthritis =polyarthrite rhumatoide
it is an inflammatory joint disorder
auto immune disease
rheumatoid arthritis is a
–chronic disease (plus 6 weeks )
–symetric (auto immune so it doesn’t spare articulations), auto antibodies doesn’t discriminate ,
joint diseases :are
–axial:spine like SPA, degenerative disc disease
–peripheral joint diseases :touch eather big joints such as polymyalgia rheumatica ,and osteoarthritis
or touch small joints such as rheumatoid arthritis +++
Rheumatoid arthritis
–inflammatory auto immune joint disease
–there are cardinal signs of inflammation such as :hotness , redness , swallen ,pain and loss of function
–symmetric (as auto antibodies doesn’t discriminate )
–worse in the morning (when we move ,we wash the inflammation so we are okay , but in the morning it is painful as the inflammation is there ))
–ESR and CRP are high ++, also ferritine can be high ,
thrombocytosis ++
–constitutional symptoms ( fever , weight loss,night sweats )

–affects small joints +++(wrists , feet , hands ,)
RA=PR
common in females ++
RA
chronic disease with acute flares
–you should know that the joint aspiration in a flare of RA will have a lot of WBC that predominantly are neutrophils PNN
NB
–diseases that are auto immune are commoner in women
–diseases that are immunodeficiency are commoner in men
rheumatoid arthritis
remember our model ROSE
who have RA , this will make you realise that RA is common in women

Néoplasie
Amaigrissement inexpliqué
Immunosuppresseur, corticothérapie
Toxicomanie intraveineuse
Infection urinaire
Ostéoporose
Lombalgie inflammatoire, d’aggravation progressive, rebelle aux antalgiques habituels
Signes généraux (fièvre, AEG),
Signes neurologiques déficitaires
