What are the two main causes of an acute sore throat?
Acute pharyngitis or tonsillitis
What are the causes of a sore throat and which is more common?
- Viral (more common) - rhinovirus, coronavirus, parainfluenza virus
- Bacterial - group A beta-haemolytic streptococcus
What is the criteria for identifying a bacterial cause of a sore throat?
Centor criteria:
- Presence of tonsillar exudate
- Presence of tender lymphadenopathy
- Presence of fever
- Absence of cough
How is tonsillitis diagnosed?
- Large, red tonsils
- May or may not be coated in a tonsillar exudate
- Tonsillar swab culture
What is the treatment for viral tonsillitis?
Reassure, symptomatic relief (ibuprofen +/- paracetamol)
What is the treatment for bacterial tonsillitis?
Symptomatic relief (ibuprofen +/- paracetamol), consider penicillin V for 10 days (macrolide for 5-days if allergic).
What is the indication for tonsillectomy in tonsillitis?
More than 6 episodes per year that are severe enough to miss school or prevent normal functioning.
What is the main complication of tonsillitis and how is it treated?
Peritonsillar abscess (Quinsy):
- Extremely painful sore throat, dysphagia
- Antibiotics and surgical drainage needed
