What are the causes of neonatal seizures?

A
  1. Hypoxic-ischaemic encephalopathy
  2. Infection (meningitis, encephalitis)
  3. Intracranial haemorrhage
  4. Structural CNS lesions
  5. Metabolic disturbance (glucose, Na+, K+, Mg2+)
  6. Neonatal withdrawn from maternal drugs
  7. Kernicterus
  8. Idiopathic (benign 5th day fits)

What is the management for neonatal seizures?

A
  1. ABCDE - turn on side
  2. Rule out/treat reversible causes
  3. Empirical antibiotics
  4. IV access - FBC, U&Es, LFTs, Ca, Mg, glucose, gas
  5. Consider cranial USS/MRI
  6. If >5 mins - 1st line phenobarbital, 2nd line phenytoin.
