What are the causes of neonatal seizures?

A

Hypoxic-ischaemic encephalopathy
Infection (meningitis, encephalitis)
Intracranial haemorrhage
Structural CNS lesions
Metabolic disturbance (glucose, Na+, K+, Mg2+)
Neonatal withdrawn from maternal drugs
Kernicterus
Idiopathic (benign 5th day fits)

What is the management for neonatal seizures?

A

ABCDE - turn on side
Rule out/treat reversible causes
Empirical antibiotics
IV access - FBC, U&Es, LFTs, Ca, Mg, glucose, gas
Consider cranial USS/MRI
If >5 mins - 1st line phenobarbital, 2nd line phenytoin.
