What are the contraindications for breastfeeding?
- HIV +ve mother
- Amiodarone
- Antimetabolites
- Antithyroid drugs
- Opiates
How long is sole breastfeeding recommended for?
First 6 months
When should you start breastfeeding and why?
- Straight after birth
2. Reduces PPH, good emotional connection.
What is the correct attachment and process for breastfeeding?
- Mouth wide open, chin touching breast, nose hardly touching.
- Lower lip curled back
- Slow, rhythmic, and deep jaw movements
- 1st few fast, shallow and non-nutritive - let down reflex
What are the options for bottle feeding and when should they be started?
- Standard infant formula (humanised cow’s milk), follow-on formula, hydrolysed formula for cow’s milk allergy.
- Start when weaning solids at 6 months
What is the target intake for bottle feeding an infant?
150ml/kg/24hr over 4-6 feeds
When should you start weaning solid foods for infants and when can you start normal cow’s milk?
- 6 months - finger fed, avoid adding salt/sugar, home cooking, eat together.
- From 1 year
When does a cow’s milk protein intolerance/allergy present and what is the pathophysiology?
- First 3 months in formula fed infants
2. Non-IgE mediated (caused by T-cells) - delayed reactions (urticaria and angioedema)
What is this a presentation of?
2 month old baby, regurgitation, vomiting, diarrhoea, blood and mucus in stools, colic symptoms (crying and pulling up legs), faltering growth, started on bottle milk.
Cow’s milk protein intolerance/allergy
How is a cow’s milk protein intolerance/allergy investigated?
- Improves with elimination of cow’s milk for 2-6 weeks then reintroduction confirms diagnosis.
- . Skin prick/specific IgE if suspecting IgE mediated.
What is the treatment for cow’s milk protein allergy?
- Hydrolysed (Similac) or amino acid formula feed (Neocate).
- Usually resolves by 1-2 years
