A
- Nature (projectile)? Colour (bilious/blood)?
- In relation to feeds?
- Worse when lying/sitting?
- Associated symptoms?
- What do they eat? Age of weaning?
- Growth? Weight loss?
- Diarrhoea? Constipation
- Behavioural changes? Sleeping well? Drowsy?

A
- Concerned about height/weight/both?
- Always been small? Birthweight?
- How has growth been since birth?
- Ill/irritable? Recent/recurrent infection?
- Wet and dry nappies? Stool (diarrhoea)?
- Any vomiting? Abdominal pain?
- Cough?

A
- Before - fever? Rash? Scared/crying? Head trauma? Watching TV/video games?
- During - duration? LoC? Shaking? Tongue biting? Incontinence? Blue/pale?
- After - how were they?
- Ever happened before?
- How are they generally? Growth and development normal?

A
- Gross motor - dominant hand, holding head up, sitting up, crawl, walking, bottom shuffle?
- Vision and fine motor - following with eyes, reaching, transferring, pincer grip?
- Hearing, speech, language - react to sounds, babble, speech?
- Social - smile, wave bye, pretend play with others?

A
- Duration? Nature? Timing?
- Productive? Colour?
- Additional noises? Pain? Breathlessness?
- Triggers? Putting things in mouth?
- URTI - headache, rhinorrhoea, sore throat, painful ears?

- Infectious contacts/smokers at home? Foreign travel?
