Acute inflammation of the middle ear secondary to either viral or bacterial infection.

Who is acute otitis media most common in and when does it peak?
A
Common in first 3 years of life, peaking in winter.

What is this a presentation of?
Rapid onset of pain, fever, irritability, often after an upper respiratory tract infection; anorexia, vomiting, bulging of the tympanic membrane.

How is acute otitis media diagnosed?
A
Inspection of the tympanic membrane with an otoscope - red and bulging.

What is the management for acute otitis media?
A
- Analgesia
- Antibiotics if systemically unwell or immunocompromised or no improvement in over 4 days.
- Amoxicillin 40mg/kg/d in 3 divided doses for 5 days (macrolide if allergic)

What are the complications of acute otitis media?
A
Mastoiditis, glue ear, perforation
