What is PROM?
Premature rupture of membranes in the absence of any uterine activity (i.e. after 37 weeks gestation).
What is the first step in managing PROM?
Admit to antenatal ward for speculum, 4-hourly temperature, and 24-hour foetal monitoring.
What should be offered for prophylaxis in PROM?
Prophylactic antibiotics.
What should be monitored intensely in PROM?
Signs of chorioamnionitis.
What management is offered if amniotic fluid is clear and 0-24 hours after rupture of membranes?
Expectant management, as 60% of women will go into labour within 24 hours.
What management is offered if amniotic fluid is clear and more than 24 hours after rupture of membranes?
Offer induction of labour (IOL).
What should be done if there is a positive group B strep in PROM?
Offer immediate induction of labour (IOL).
What should be done if meconium is present in PROM?
Induce labour as soon as possible.
How should the neonate be monitored after delivery in PROM?
Monitor for at least 12 hours after delivery, measuring temperature, HR, and RR at 1, 2, 6, and 12 hours.
