Magnesium toxicity:
Signs of magnesium toxicity include respiratory depression, absent DTRs, decreased LOC,
and oliguria. If magnesium toxicity is present, stop the magnesium infusion and administer the antidote
calcium gluconate.
Oxytocin:
Oxytocin is contraindicated for clients with a previous classical cesarean incision due to the risk
for uterine rupture. Monitor clients receiving oxytocin for uterine tachysystole

Considerations

Indication: Promote fetal lung maturity if
<34 weeks gestation and at risk for preterm labor.
Mechanism: Corticosteroid that stimulates surfactant
production in fetal lungs
Nursing considerations
Administer IM to the mother in 2 doses, 24 hr apart.
Monitor for maternal hyperglycemia.
Rh-negative mothers
Mechanism: Suppresses the immune response to
Rh-positive fetal blood cells
Nursing considerations
Administer at 28 weeks and again within 72 hr
postpartum if baby is Rh-positive and mother
is Rh-negative
Erythromycin Considerations
● Apply a 1- to 2-cm ribbon to
both eyes from inner to outer
canthus within 1 hr of birth.
● Eye irritation is expected.
Vitamin K Considerations
● Administer IM within 1 hr
of birth in vastus lateralis
muscle.
● Erythema and edema at the
injection site are expected.
Hepatitis B
Vaccine
Considerations
● Administer IM before
discharge.
● If mother is HBsAg-positive,
administer with HBIG within
12 hr of birth.
