Heart Failure
Left HF
Left-sided HF causes pulmonary edema (crackles, dyspnea),
right HF
right-sided HF
causes peripheral edema, JVD, and hepatomegaly.
BNP: B-type natriuretic peptide (BNP)
is secreted
in response to ventricular stretching. Elevated
BNP indicates fluid overload.
Diet education
Clients with HF should restrict
fluids to <1.5-2 L/day and sodium to 2 grams/
day. To reduce sodium intake, limit intake of
packaged and processed foods.
Daily weights:
Clients with HF should monitor
their weight at the same time daily, using the same scale and wearing the same clothes.
Report a weight gain of >3 lb (>1.5 kg) in 2 days or >5 lb (>2.5 kg) in 1 week.
Pulmonary edema:
Monitor a client with HF for crackles or pink, frothy sputum, which indicate flash
pulmonary edema. If a client develops acute pulmonary edema, position in high-Fowler and administer
oxygen and IV diuretics.
