Hypothyroidism
Hypothyroidism causes low T3 and T4 levels,
high TSH levels, and decreased metabolic
activity.
hyperthyroidism:
Hyperthyroidism causes high T3 and T4 levels, low TSH levels, and increased metabolic activity.
Levothyroxine monitoring:
Levothyroxine
is a lifelong thyroid hormone replacement.
Instruct clients to report chest pain, palpitations, or HR >100 and to never abruptly stop the medication to prevent
myxedema coma.
Do not palpate goiter:
Do not palpate
a goiter in clients with hyperthyroidism.
Palpation causes excess thyroid hormone secretion and thyroid storm.
Radioactive iodine teaching:
Following RAI
therapy, reduce radiation exposure by avoiding
close contact with pregnant women and
children and flushing 2-3 times after toileting.
Thyroidectomy care:
After a thyroidectomy, position the client in the semi-Fowler position to decrease
airway edema and keep a tracheostomy kit at the bedside. Trousseau sign indicates hypocalcemia from
parathyroid gland damage, and high fever indicates thyroid storm.
