Q

Nursing considerations:
Chorioamnionitisrisk for sepsis in the mother and fetus.

A

Prevention and monitoring:
Minimize vaginal exams after membranes rupture.
Maintain sterile technique during labor
procedures (cervical exams, internal monitoring
placement, amniotomy).
Check maternal and fetal vital signs frequently for
signs of chorioamnionitis (HR, maternal fever).
Palpate for uterine tenderness.
Assess for foul-odored discharge.

Q

Treatment:

A

Obtain CBC and blood cultures as prescribed.
Administer broad-spectrum IV antibiotics as
prescribed (ampicillin, gentamicin).
Prepare for cesarean delivery if indicated
(dysfunctional labor, worsening infection,
fetal distress).

Q

Risk factors:

A

Prolonged rupture of membranes (>18 hr)
Frequent cervical exams during labor
Internal monitoring devices (fetal scalp electrode,
intrauterine pressure catheter)
Genital tract infections (chlamydia)

Q

Chorioamnionitis prevention:

A

To prevent
chorioamnionitis, minimize vaginal exams
after membranes rupture and maintain sterile
technique during labor procedures.

Q

Neonatal Sepsis

A

Neonatal sepsis = systemic infection in the newborn.
Pathophysiology:
Bacteria (GBS, E. coli) enter the bloodstream 
Overwhelms the neonatal immune system 
Systemic inflammation
