Paget’s disease is bone deformities with abnormal mixture of lamellar and woven bone because of initial high osteoclast activity followed by heightened osteoblast due to abnormal bone breakdown and subsequent formation, with a high ALP
What are the key diagnostic findings for Paget’s?
High ALP; Norma calcium
X-ray for early findings of osteolysis
What are the risk factors for Paget’s?
Age over 40
Family history
Anglo-Saxon descent
What are key symptoms of Paget’s disease?
Bonds deformity like frontal bossing
Bones can expand and compress nerves causing hearing loss and cranial nerve palsies
Deep boring pain in affected bones
Pathological fractures
What are the clinical findings for Paget’s disease?
Bounding pulse and tachycardia due to high output
Positive Tinel and Phalen’s due to carpal tunnel
Collapsing pulse
Bossed skull
Visual loss, cranial nerve abnormalities
Hearing aids
Features of heart failure
Bowing of the legs/bony deformity
Hot skin overlying the bone involved
What is the management of Paget’s disease?
Pain management with analgesics
She of bisphopshiates to inhibit osteoclastic activity
