What is used to test for H.pylori?
Urea breath test
Stool antigen test
When should testing be avoided?
within 2 weeks of treatment with a proton pump inhibitor or within 4 weeks of antibacterial treatment, as this can lead to false negatives.
When should retesting occur?
Ideally 8 weeks after treatment regime, where there are persistent symptoms. The retest should use urea breath test Stool
What is the drug treatment for H.pylori?
Triple therapy with proton pump inhibitor
Omeprazole/Lansoprazole
AND two antibacterial
Amoxicillin AND Clarithromycin/ metronidazole
What should be used if initial treatment is not successful?
AFTER 7 DAYS
PPI
Amoxicillin and use either of clarithromycin or metronidazole which was not initially used
What is alternative to clarithromycin and metronidazole second line?
PPI and amoxicillin and tetracycline
What is third line?
PPI and bismuth and clarithromycin/tetracycline
