INR Target Flashcards
What is the target INR for mitral valve repalcement?
2.5 to 3.5
What is the target INR for patinets with AFib?
2-3
What is the target INR for aortic valve replacement?
2-3
What is the target INR for patinets following VTE?
2-3
What is the target INR for patinets with VTE while on warfarin?
3-4
Which vessels are anticoagulants less useful?
Anticoagulants are of less use in preventing thrombus formation in arteries, for in faster-flowing vessels thrombi are composed mainly of platelets with little fibrin to act as a target
How long do vitamin K antagonists take to work?
48 to 72 hours
What should be given for immediate anticoagulation?
LMW heparin
Unfractionated heparin
When should oral anticoagulants be avoided as first INR therapy?
Peripheral artery occlusion
Cerebral artery thrombosis
What should be done with warfarin in patients with major bleeding?
Stop warfarin
Give slow infusion of Vitamin K and either prothrombin complex or fresh frozen plasma
What is used as a bridging therapy for warfarin?
LMWH therapy
What should be done if warfarin fails to reach target INR after bridging therapy?
Increase warfarin dose to find ideal dosing regimen
Restart LMWH bridging therapy until adequate anticoagulation occurs
How is warfarin induction dose obtained?
LMWH bridging therapy
Warfarin induction dose for 2 days
Day 3 to confirm subsequent dose
What is the target INR for patients with recurrence of VTE while on warfarin?
3-4
