When is endovascular techniques used in PAD management?
For short segment stenosis less than 10cm, ideal for HIGH risk patients. This invovles ballon angioplasty and/or stenting.
When is an open surgical technique used?
For long segment lesions over 10cm and multi focal lesions because it indicates multi focal lesions, ideal for lower risk patients and involves creating:
a surgical bypass
endartectomy.
What type of imaging is used first INR?
Duplex ultrasound
-> second line is magnetic resonance angiography
What is the pharmacological management of PAD?
Atorvostatin 80mg regardless of total cholesterol
Clopidogrel antiplatelet
What is a Marjolin’s ulcer?
Squamous cell carcinoma that develops in areas of chronic skin injury or inflammation with old burn scars or osteomyelitis. It occurs mainly on lower limb.
Treatment is with surgery.
