What is autoimmune gastritis?
Rarest form
Anti-parietal and anti-intrinsic antibodies are present which lead to atrophy and intestinal metaplasia in the fundus and body of stomach
Possible complications of autoimmune chronic gastritis?
Pernicious anaemia, macrocytic due to B12 deficiency
Increased risk of malignancy
Due to Vit B12 deficiency there is risk of subacute degradation of the spinal cord
Bacterial Chronic gastritis which bacteria causes it?
H. Pylori
What is bacterial chronic gastritis?
Most common type
-Bacteria inhibits a niche between the epithelial cell surface and mucous barrier. This excites an early acute inflammatory response, if the bacteria is not cleared then there is chronic active inflammation.
What type of bacteria is H. Pylori?
Gram negative curvilinear rod
How is chronic bacterial gastritis diagnosed?
Presence of H. Pylori ABs produced by lamina propria - follow H. Pylori guidance
What does chronic bacterial gastritis increase the risk of?
- Duodenal cancer
- Gastric ulcer
- Gastric carcinoma
- Gastric lymphoma
- Can lead to gastric intestinal metaplasia, a precursor for gastric cancer
What is chemical chronic gastritis?
Marked epithelial regeneration, hyperplasia, congestion and little inflammation
Who gets chemical chronic gastritis?
NSAID users
Alcohol
Bile reflux
Direct injury to muscle layer by fat solvents
Complications of chemical chronic gastritis?
Can produce erosions or ulcers
