category A
adequate, well-controlled studies in pregnant women have not shown an inc. risk of fetal abnormalities to the fetus in any trimester
Category B
animal studies have revealed no evidence of harm to fetus
no adequate or controlled studies in pregnant women
OR
animal studies have shown an adverse effect, but studies in pregnant women have failed to demonstrate risk
Category C
animal studies have shown an adverse effect and there are no controlled studies in pregnant women
OR
no animal studies have been conducted and there are no studies in pregnant women.
Category D
adequate well controlled or observational studies in preg women have demonstrated a risk to the fetus
- benefits vs risk
- drug may be acceptable if needed in a life-threatening situation or serious disease
category X
adequate well controlled or observational studies in animals or preg women have demonstrated positive evidence of fetal abnormalities or risks

patient identification date and time allergies full name of med dose frequency reason for PRN duration of therapy
outpatient Rx
provider: name, office address, phone number pt name, address, date direction for the pt or pharmacist name of drug and quantity refills if required signature

- heroin, MDMA, LSD, marijuana
schedule 2 drugs
hight potential for abuse
currently accepted medical uses
abuse may -> sever psychological or physical dependence
- morphine, fentanyl, methadone, methylphenidate, amphetamines, oxycodone
schedule 3 drugs
potential for abuse, less than class 1 &2
accepted medical use
abuse: moderate-low physical dependence, high psychological dependence
- buprenophine, codeine, dronabinol (marinol), testosterone
Schedule 4 drugs
low potential for abuse relative to class 3
accepted medical tx
abuse: limited physical dependence or psychological dependence relative to schedule 3 drugs
- benzodiazepines, zolpidem, phenobarbitol, modafinil, tramadol
schedule 5 drus
low potential for abuse relative to schedule 4 drugs
accepted medical tx
abuse: limited physical dependence or psychological dependence relative to schedule 4 drugs
- cough syrups w/ small codeine, pregabalin, antidiarrheals w/ opium or diphenoxylate
