If TS Safety Limit 2.1.1(DNB) is violated, what needs to be done?
Required to be in Mode 3 w/in 1 hr
What are the assumptions of Final Safety Analysis Report (FSAR)?
- One train of ECCS is assumed to fail
- One instrument failure would neither cause or prevent ECCS actuation
- One accumulator is lost through a break
What does Surveillance Requirement 3.0.2 state in regards to Grace?
Grace period of 25% (Drop dead due date)
What does Surveillance Requirement 3.0.3 state in regards to a missed surveillance?
Must meet within 24 hrs or normal surveillance frequency, whichever is greater
What does LCO 3.0.3 state:
1. When an LCO is not met and the associated ACTIONS are not met, an associated ACTION is not provided, or
2. if directed by the associated ACTIONS, the unit shall be placed in a MODE or
3. other specified condition in which the LCO is not applicable.
Action shall be initiated within 1 hour to place the unit, as applicable, in:
a. MODE 3 within 7 hours;
b. MODE 4 within 13 hours; and
c. MODE 5 within 37 hours.
What is the purpose of the Reactor Core DNB Safety Limits?
Prevent overheating of the fuel and cladding, which would result in release of fission products to the RCS if not prevented
What is the purpose of the RCS pressure Safety Limits
Prevents over pressurization of the RCS and maintains integrity which then prevents the release of radionuclides contained in the RCS from reaching the containment atmosphere
Can you extend a Surveillance Requirement if its the initial surveillance after the initial event?
No, you can only extend 25% for subsequent surveillances after the initial surveillance has been complete
The amount of time allowed for completion of a Required Action is the definition of?
Completion Time
Required Actions must be completed prior to the expiration of the specified Completion Time
Equipment is assumed to be Operable if its what are met?
If its SRs are met
Equipment is inoperable if?
Ref SR 3.0.1
Equipment is Inoperable if
- It is known to be inoperable, even if the SRs are current
- An SR is known not to be met, even if the SR is between its required performance interval
Surveillance Requirements do not have to be performed when?
when the associated LCO is not applicable, unless specifically stated, or on inoperable equipment or variables that are outside limits
Do Surveillance Requirements have to be performed following maintenance prior to returning equipment to Operable status?
Yes, unless otherwise stated
