E. Clonorchis Sinensis-chinese liver fluke would get it from eating raw fish, increases of cholecystitis or adenocarcinoma of the bile duct

Dx is Whipple’s Disease

TB also stains PAS positive but would not cause the sx described.

Whipple’s disease can also cause CNS issues.
