A 54 year old man comes to his doctor with complaints of abdominal pain and fatigue. Examination is done showing a massive spleen and blood test show CD103+ cells. What else would you expect on examination?
A. 9:22 translocation B. TRAP positive C. Smudge cells D. Low LAP E. Defective RAR
RFA
ans. B TRAP positive- patient has Harry Cell Leukemia( C103+, massive splenomegaly)
A. 9:22 translocation- CML, or bad prognosis ALL
C. Smudge Cells- CLL
D. Low LAP- CML
E. Defective RAR-AML
A 40 year old man goes to the doctor for a check up he has blood work done. The lab is notable for CD5+ B cells that are cyclin D1 negative. WBC is 4300. What is the man’s diagnosis?
A. T- Cell Leukemia B.Chronic Lymphoblastic Leukemia C.Small Lymphocytic Lymphoma D. Mantle Cell Lymphoma E. Hairy Cell Leukemia
RFA
Ans. C Small Lymphocytic Lymphoma- is CD5 B cells positive that are cyclin D1 negative, with WBC less than 5000.
A. T-Cell Leukemia- While there would be CD5 cells they would be T cells not B cells, this condition is very related to infection of HTLV 1
B. Chronic Lymphoblastic Leukemia would have CD5 positive B cells it would also be cylind D1 negative but WBC would be greater than 5000
D. Mantle Cell Lymphoma- It would have CD 5+ B cells but they would be cyclin D1 positive, ( related to a 11:14 translocation)
E. Hairy Cell Leukemia- would be CD103 positive
