Superior Mesenteric Artery Flashcards
Origin
From front of abdominal aorta opposite 1st lumbar vertebra lower border (about 1cm below coeliac trunk)
Course
- Lies behind pancreas body between two veins (splenic vein (above artery origin) and left renal vein (below artery origin))
- Descends in front of pancreas uncinate process and duodenum 3rd part
- Enter root of small intestine mesentery
- In mesentery, it runs downwards and to right (with superior mesenteric vein on its right) crossing over 6 structures
- Surrounded by superior mesenteric plexus of nerves throughout its course
Structures crossed over
- Abdominal artery
- IVC
- Right Psoas major
- Right ureter
- Right gonadal vessels
- Right genitofemoral nerve
Termination
At ileocaecal junction by anastomosing with ileocolic branch
Branches
- Middle colic
- Inferior pancreatico-duodenal
- Right colic
- Ileocolic
- Jejunal
- Ileal
Middle colic site
Arises just below pancreas and runs in transverse mesocolon dividing into right and left branches
Right branch
Anastomoses with ascending branch of right colic artery
Left branch
Anastomoses with ascending branch of superior left colic artery
Supply
Middle 1/3 of transverse colon
Inferior pancreatico-duodenal site and course
- Arises as it crosses uncinate process
- Runs in groove between pancreas head and duodenum
- End by anastomosing with superior pancreaticoduodenal artery (from hepatic artery)
Inferior pancreaticoduodenal supply
- Duodenum lower 1/2
- Lower 1/2 of pancreas head and its uncinate process
Right colic course
- Arises from the artery concave right side at its middle
- Runs to right retroperitonealy towards ascending colon
- Crosses 3 structures
- Ends by dividing into ascending and descending branches
Right colic crossed structures with
- Right gonadal vessels
- Right ureter
- Right Psoas major
Ascending branch right colic
Anastomoses with right branch of middle colic artery
Descending branch right colic
Anastomoses with ascending branch of ileocolic artery
Right colic supply
- Upper 2/3 of ascending colon
- Right colic flexure
Jejunal and Ileal
- 12-15 branches arise from the artery convex left side
- Run between 2 layers of small intestine mesentery
- Divide repeatedly forming arches (arterial arcades) which give straight end arteries (vasa recta)
- Jejunal branches are longer and less numerous their arcades are fewer than Ileal branches
Ileocolic course
- Arises from lower part of the artery concave right side
- Runs to right retroperitonealy towards right iliac fossa
- End by dividing into ascending and descending branches
Ascending branch ileocolic
Anastomoses with right colic artery
Descending branch ileocolic
Runs towards ileocaecal junction to anastomose with superior mesenteric artery termination
Descending branch ileocolic branches
- Ascending branch
- Anterior and posterior Caecal artery
- Appendicular artery
- Ileal branch
Ascending branch of descending branch supply
To ascending colon lower 1/3
Anterior and posterior Caecal arteries supply
To caecum front and back

Descends behind ileum termination to enter mesoappendix
