Subphrenic spaces
- Potential spaces between diaphragm and transverse colon
- May be pushed collection sites forming subphrenic abscess
Liver and its peritoneal folds divide Supracolic compartment into 3 subhrenic spaces on each side of falciform ligament
2 intraperitoneal
1 extraperitoneal
1. Right anterior intraperitoneal space
2. Right posterior intraperitoneal space (Morison’s or hepato-renal pouch)
3. Left anterior intraperitoneal space
4. Left posterior intraperitoneal space
5. Right extraperitoneal space
6. Left extraperitoneal space
Right anterior boundries
- Lies between liver right lobe and diaphragm on falciform ligament right side
- Closed posterior by coronary ligament upper layer and right triangular ligament anterior layer
Right anterior boundries
- Lies between liver right lobe and diaphragm on falciform ligament right side
- Closed posterior by coronary ligament upper lay and right triangular ligament anterior layer
Right anterior importance
Can be infected from:
1. Gall bladder
2. Liver
3. Stomach
Or
4. Duodenum
Diseases
Right posterior boundries
- Anterior: inferior surface of liver right lobe
- Posterior: right kidney anterior surface
- Coronary ligament lower layer
- Opens into general peritoneal cavity
Right posterior importance infection
Can be infected from:
1. Gall bladder
2. Duodenum
3. Subhepatic appendix
4. Colon
Or
5. Kidney
Right posterior importance site
Commonest site for subphrenic abscess because it is peritoneal cavity most dependent area during lying down
Left anterior boundries
- Lies between liver left lobe and diaphragm on falciform ligament left side, anterior abdominal wall, and stomach Anterosuperior surface
- Closed posterior by left triangular ligament anterior layer
Left anterior
Can be infected after:
1. Splenectomy
2. Gastrectomy
Or
3. Anterior gastric perforation
Left posterior boundries
It is lesser sac upper part
Left posterior importance
Can be infected from:
1. Stomach
2. Pancreas
3. Liver
Or
4. Splenic

Right Extraperitoneal space boundaries
Lies between liver bare area and diaphragm bounded by coronary ligament upper and lower layers
Right Extraperitoneal space importance
Can be infected from:
1. Kidney
2. Liver
Or
3. Pleural
Diseases
Left Extraperitoneal space boundary
Lies around left Suprarenal gland and left kidney upper pole
