Portal Vein Flashcards
Beginning
Behind pancreas neck by union of splenic and superior mesenteric veins
Course
- It ascends behind duodenum 1st part
- Enters lesser omentum free margin
- Ends in porta hepatis where it divides into right and left branches
Has no valves, it allows blood passage in the 2 directions
Relations in duodenum 1st part
- Anterior: duodenum 1st part separated by common bile duct (anterior and to right) and gastroduodenal artery (anterior and to left)
- Posterior: IVC
Relations in lesser omentum free margin
- Anterior: common bile duct (anterior and to right) and proper hepatic artery (anterior and to left)
- Posterior: IVC (separated from it by epiploic foramen)
Relations in porta hepatis
- Anterior: hepatic artery
- Posterior: caudate process of liver caudate lobe (separates it from IVC)
Size
3 inches long
Up to 12 mm in diameter
Tributaries
Beginning:
1. Splenic vein
2. Superior mesenteric vein
Termination:
3. Paraumbilical veins
4. Cystic vein
Join main trunk:
5. Right gastric vein
6. Left gastric vein
Splenic vein beginning
At spleen hilum by union of 5-6 splenic tributaries
Splenic vein course
- Leaves spleen
- Passes through lienorenal
- Continuous to right behind pancreas body and in front of:
- Left kidney and it’s hilum
- Left sympathetic trunk
- Left crus diaphragm
- Abdominal aorta (but separated from it by superior mesenteric artery)
Splenic vein termination
Behind pancreas neck by joining superior mesenteric vein forming portal vein (not tortuous (unlike splenic artery)
Splenic vein tributaries
- Splenic
- Left gastroepiploic
- Inferior mesenteric
- Short gastric
- Pancreatic
Splenic
5-6 tributaries from spleen
Short gastric
From stomach fundus
Left gastroepiploic
From stomach greater curvature
Pancreatic
From pancreas body
