A
  1. Ruptured appendix
  2. Perforated ulcer
  3. Postoperative rupture of breakage of anastomosis in abdominal/pelvic cavity
  4. Perforation from endoscopic procedures
  5. Penetrating trauma to abdomen

  1. Abdominal pain
  2. Rebound tenderness
  3. Muscular rigidity
  4. Spasms
  5. Shallow respirations due to pain
  6. Abdominal distention
  7. Fever
  8. N/V
  9. Tachpnea, tachycardia

  1. Postoperative and post endoscopic procedure vital signs and abdominal assessment
  2. Pain assessment
  3. Maintain patent IV line and administer orderered antibiotics
  4. Drain care and management
  5. NPO and main need NG
  6. Accurate I/O and Monitoring of electrolytes
