Nursing Dx: Knowledge Deficient: Self-care needs r/t new needs secondary to surgical procedure
**Patient Outcomes: **
- States S/Sx of infection and venous thrombosis prior to discharge
- Describes activity restrictions prior to discharge
- Describes therapeutic effects, possible side-effects, and dosing schedule of discharge medication prior to discharge (pain, prophylactic antibiotics, estrogen)
- Describes appropraite incision care prior to discharge
Nursing Interventions:
- Teach S/Sx of venous thrombosis
- Teach activity restrictions (vaginal rest, avoid vaginal intercourse X 4-6 weeks, no tampons, no douches, and avoid any heavy lifting for 2 months)
- Teach immediate surgical menopause will result due to loss of both ovaries
- Teach S/Sx of infection, incision care
- Medication teaching prn for all discharge medications

Abdominal/Perineal Resection
A
- Abdominal incision and perineal incision made to remove tumor low in the large intestines
- Drains are usually placed at each site
- A colostomy is usually performed

Lower Anterior Resection (LAR)
A
- Used to remove a tumor anywhere in the large intestines, except the terminal rectum
- Abdominal incision is utilized and a JP drain is usually present
- Ostomy is usually not performed in early stages of colon cancer

A
Portions of ascending colon and transverse colon removed. Colon is then connected to end of small intestine
