How to measure a client for crutches
The client lies in a supine position and the nurse measures from the anterior fold of the axilla to the heel of the foot and adds 2.5cm (1 inch)
OR
The client stands erect and positions the crutch. The nurse makes sure that the shoulder rest of the crutch is at least 3 finger widths (2.5-5cm) below the axilla
Teaching for crutches
- Develop a plan of exercises to strengthen the arm muscles
- Instruct on appropraite crutch gait
- Getting into/out of chair/bed
- Getting into/out of car
- Going up/down stairs
- Keep crutch tips dry and clean
- Wear a shoe with low heel that grips the floor

Four Point Gait Uses
Safe, slow. Use when weight-bearing is allowed for both legs
Two Point Gait
L. crutch and R. foot move together, then R. crutch and L. foot together
Two Point Gait Uses
Faster, safe. Requires at least partial weight bearing on each foot
Three point Gait
Weaker leg and both crutches move together, then move stronger leg.
Three Point Gait Uses
Faster, safe. Must be able to bear the entire body weight on the unaffected leg
Swing-To Swing-Through Gait
Advance crutches together then bring both feet to or through the crutches.
Swing-to Swing-Through Gait Uses
Fast gait, balance and stength required. Used by clients with paralysis of legs and hips
