Precursor to melanoma
Atypical nevus
- DEPTH of invasion –> mets
Most common skin cancer, pallisading cells, pearly pink papule with telangiectasias, upper lip, little risk for mets
BCC
Red, scaling/crusty, indurated/ulcerated lesion, on lower lip, arms, back, hands, little risk for mets, sheets and islands of cells with keratin pearls
SCC
- old age, fair skin, p53
Precursor to SCC
Actinic keratosis & Bowen disease (SCC-IS)
“Stuck on,” papillomatous epithelial proliferation of basaloid cells w/ horn cysts
Seborrheic keratosis
Pruritic papules and plaques with fine white line that is lace-like over violet skin on the flexor surface of the wrists
Lichen Planus
- 5Ps of Planus
- “Wickham’s striae”
Rough, scaly papule w/ hyperkeratosis, parakeratosis, atypical keratinocytes
Actinic keratosis
Vesicular lesions on extensor surfaces
Dermatitis herpetiformis = Celiac
Pruritic target lesions on palms and soles
Erythema multiforme - HSV > mycoplasma - drug reactions \+ fever and mucosal = SJS \+ sloughing = TEN
Blisters on dorsal hands, hypertrichosis, hyperpgimentation, elevated serum and urine porphyrin, no abdominal pain
Porphyria cutanea tarda
- HCV, HIV, INC IRON, estrogen
- deficiency in uroporphyrinogen decarboxylase
Sawtooth histology, HCV
Lichen planus
Thrombosis of superficial veins that appear, resolve and reappear elsewhere on the body
- Trousseau’s sign (superficial migratory thrombophlebitis)*
- pancreatic cancer
Dry scales, with flaking, pruritus on scalp, axilla, external ear
seborrheic dermatitis (dandruff) - Tx = selenium sulfide or hydrocortizone
Flushing of face with spicy foods, papular lesions, conjunctivitis, rhinophyma (large red nose)
Acne rosacea
- metronidazole gel
- oral tetracycline
