Classification Systems (ICD-10) Flashcards
What is the ICD
International statistical classification of diseases and health related problems
Monitor global mortality and morbidity statistics
Data collected from many countries
What is schizophrenias code number
F20-29
Whats hebephrenic schizophrenia
F20.1
Label F30.0.01
F = mental health section
3 = family of disorder (schizophrenia)
0 = specific disorder
.0 = severely mild
.01 = physical symptoms
How do you make a diagnosis
1) Select key words from interview relating to symptoms
2) Look up symptoms in alphabetic index or go obvious section
3) Use other symptoms to locate subcategories (F20)
3 Improvements to the ICD-10
1) Presentation, communication, interpretation of symptoms shaped by language and culture
2) Research diagnosis needs to be taken from culture of patients
3) Inconsistencies, ambiguities and overlaps between disorders that were removed from ICD so it’s clearer, simpler more organised
Reliability -> improvements
ICD-9 and ICD-10
Ponizovsky et al (2006) compared between the two
Positive predictive value scores for schizophrenia increased from 68% to 94.2%
CA of improvements of reliability
High reliability meaningless without high level of validity
Demonstration of high stability means nothing if reliability tells us true meaning of diagnosis
Galezzi et al (2004) reliability
2 researchers carried joint interviews with 100 clients
Looked at psychosomatic symptoms with high agreement on results
Encourages confidence in using ICD-10 (for at least some disorders)
Validity strength
Predictive for schizophrenia
Mason et al (1997) compared different wqays of making a diagnosis - both good at predicting disability in 99 people with schizophrenia 13 years later
Application
WHO aims to improve clinical utility of system in ICD-11 by conducting research
Survey found preference for simplicity and flexibility so is not cautious to add disorders
System more user friendly and diagnosis accuracy should improve
