GnRH characteristics?
GnRH therapy increases LH and FSH in anterior pituitary
Used for Kallman syndrome or when there isnt GNRH
Can be used in infertility
Leuprolide
GnRH analog
Combinations of FSH, LH, and CG are used to increased
sperm counts in men and for follicle maturationin women.
FSH can be used alone but is more effective when used
with LH or CG. LH and CG are ineffective alone
Uses - 1. Prostate Cancer. 2. Central progressing
precocious puberty (Mccune Albright
syndrome, Granulosa cell tumor, CAH). 3.
Breast Cancer
Reduce susceptibility to degradation

ER partial agonist; antagonizes estrogen in the breast,

ER pure antagonist - Breast cancer
Clomiphene
ER partial agonist; inhibits estradiol binding to receptors
in anterior pituitary and hypothalamus, causes less
negative inhibition and more FSH and LH release from
anterior pituitary. LH and FSH surge allows for ovulation
to happen
Fertility drug
Anastrazole or Letrozole
Inhibits estrogen synthesis by inhibiting aromatase;
blocks the aromatization testosterone -> 17B estradiol;
Androstenedione -> estrone
Used when Tamoxifen cant Breast cancer
Mifepristone
“Abortion Pill.” Prevents progesterone from binding its
receptor and does not allow maintenance of a pregnancy.
1. Decreased endometrial development and secretions. 2.
Decreased lactation. 3. Decreased thermogenic effect
Combination: synthetic estrogen and synthetic
progestin
Estrogen + Progestin combo = strong negative feedback.
Decreased FSH and LH and prevention of LH, FSH
surge. Prevents follicular development and prevents
ovulation. Also thickens cervical mucous
Depo Provera -
Medroxyprogesterone
Decreases gonadotropins. Suppresses ovulation and
thickens cervical mucous
Vaginal ring (Nuvaring)
Etonogestrel makes is tougher to implant!
Contains 15 mg of Ethinyl estradiol and 120 mcg of
etonogestrel (sythetic desogestrel). Insert ring into vagina
and it stays for 3 weeks. Suppresses ovulation. Thickens
Cervical mucus. Prevents endometrial thickening
Continuous
Progestin (only)
“Minipill”
Increases cervical viscosity, endometrial thickness, and
fallopian tube mobility, reducing sperm penetration. Does
not inhibit the mid-cycle gonadotropin surge.
Seasonale/ Seasonique
3 month supply of combined estrogen and progestin
followed by 7 placebo pills which allows menses.
Plan B - Levornogestrel
Inhibits ovulation, disrupts follicular development,
interferes with maturation of corpus luteum
Pregnancy rate 15/100
Flutamide and Biclutamide
Competitive inhibitors of AR and inhibits both
testosterone and DHT binding
