What is an exanthem?
The cutaneous manifestation of a systemic infectious disease
What are the six classic infectious agents that cause exanthems?
- Measles
- Rubella
- Roseola
- Varicella
- Scarlet fever
- Erythema infectiosum

Parvovirus B19

What is the first sign of measles?
Koplik spots—white papules on red base on the buccal surfaces
Describe the rash of measles.
Erythematous maculopapular rash that progresses cephalocaudally and then regresses cephalocaudally
What are the complications of measles?
- pneumonia
- encephalitis
- subacute sclerosing panencephalitis
Describe the rash of rubella.
Fleeting, discrete macules
When is rubella infection particularly dangerous? l
Infection in pregnant women causes embryopathy in the fetus.
Describe the natural course of roseola.
High fever for 3 days followed by a rash that appears when the fever drops
What is the progression of the rash in roseola?
Starts on the trunk and spreads to the extremities
What is the treatment of roseola?
Supportive care
What is the classic rash of fifth disease?
“Slapped cheeks” and lacy rash (trunk → arms/legs)
What is the classic rash of varicella?
Crops of vesicles (dew drops on a red base) beginning on trunk
