Dental charting Flashcards
importance of charting
accurate record of dental condition, communication with colleagues, medio-legal issues, forensics
What has charting been used for in forensic dentistry?
identifying bodies and murderers, crime
What should be charted?
caries, dentures, crowded/rotated teeth, missing teeth, restorations, splint, veneers, bridges
What do you need to include when charting restorations?
type of material - AM, COMP, AU
Difference between periodontal disease and periodontitis
Periodontal disease is a broad term for diseases of the gingivae and supporting tissues of the teeth, whereas periodontitis is the inflammation of the supporting tissues of the teeth, resulting in irreversible tissue destruction.
how do you identify pockets?
probing
Plaque retention factors
calculus, overhanging restorations, poor margins on dentures/crowns…
What does BPE stand for?
Basic Periodontal Exam
How many segments is the mouth split into during a BPE?
6
What should you chart first?
missing teeth
what does FDI stand for?
Federation Dental International
How does FDI deciduous charting work?
Starts at UR with 5,6,7,8 (LR). second number is still tooth number (1-5)
How does Palmer charting work?
Using drawing of corner with tooth number inside. 1-8 for permanent teeth, A-E for deciduous.
What to draw if there is caries and a restoration is needed?
Hollow circle on surface with caries. or circle and triangle for interproximal caries/extensions
if a restoration has been completed what should you draw?
Fill in the hollow circle / circle triangle
if a restoration requires replacing, what should you draw?
line around restoration
why would a restoration need replacing?
caries, lost restoration, fractured
How to indicate a buccal cervical restoration (down at gingiva)?
filled in semi-circle at edge of box
What to do if there are 2 separate occlusal restorations on the same surface?
draw two separate filled-in circles
What to do if there is a partially erupted tooth?
PE under/over box
what to do is there is an unerupted tooth?
UE
How would you confirm the presence of an UE tooth?
Radiograph
how to indicate the presence of a fissure sealant?
FS
How to indicate if a tooth has drifted distally/mesially?
Horizontal arrow over box in the direction
how to indicate a tooth outstanding or instanding?
Vertical arrow in correct direction
How to indicate if a tooth needs extracting?
diagonal line across box
how to indicate if a tooth has now been extracted? (in that appointment)
X in box (draw second diagonal line)
how to indicate that a tooth is missing? (e.g. appointment after an extraction)
Horizontal line across box
how to indicate a fractured incisal edge?
#
how to indicate the presence of a crown
bold outline of box lines
how to indicate retained roots
+ (number of + equivalent to number of roots present/should be present if uncertain)
How to indicate a temporary dressing/filling?
Vertical stripes in circle/triangle
How to indicate if root canal treatment is required due to cavity?
Draw a hollow circle at the surface of the tooth where caries is present. write RCT under/over tooth
How to indicate a root canal filling with a complete restoration?
Fill in circle that indicates caries. RCT should still be written there.
What do you call the structures on the teeth adjacent to the missing teeth that are supporting the bridge?
Bridge retainer (BR)
What do you call the structure that replaces missing teeth in a bridge?
Bridge pontic (BP)
How to indicate the presence of a bridge
Draw a thickened edge around the crown and write BR and the material used (e.g. MCC). Draw a horizontal line across the missing tooth and write BP
How to indicate a splint or retainer
Double ended horizontal arrow above/below boxes
How to indicate a porcelain veneer?
PV written above/below box
How to indicate a missing tooth with the space closed (ortho)
Double headed arrow across box
How to indicate a retained deciduous tooth?
Box off a corner of the tooth and write C in the small box created.
Why might a deciduous tooth be retained?
In the case of hypodontia where a lower number of teeth develop compared to normal.
