What are heartburn sensations caused by?
GERD - leakage of gastric acid into esophagus
peptic ulcers =
ulcerations of mucosal lining of
- lower esophagus
- stomach
- duodenum
What happens with chronic peptic ulcers?
- destroy musculature
- replace it with permanent scar tissue
Why are chronic peptic ulcers dangerous if untreated?
can erode into the stomach and spread infection
Causes of peptic ulcers
- aggressive action of hydrochloric acid and pepsin on the mucosa
- infection of Helicobacter pylori
- Chronic NSAID use
Peptic ulcers: This requires further screening and possible medical referral (PMH)
PMH of peptic ulcer with new onset back or shoulder pain
What are stress ulcers?
stress related mucosal disease occurs in response to SIGNIFICANT physiological stress
(not just life stress)
Example of stress ulcers
gastric mucosal changes develop within 72 hrs in 80% of pts with significant burns
stress ulcers: pain presentation
- differ pathologically
- usually pain-free until perforation and hemorrhage occur
Stress ulcers: mechanism?
- likely involves ischemia of gastric mucosa
- has large O2 requirements and high acidity
s/s peptic ulcers
- “heartburn”
- night pain
- radiating back pain
- stomach pain
- R shoulder pain
- N/V
- bloody stools
