the structural changes in the airway wall occur in parallel with inflammation meaning that more inflammation equals more airway remodelling
when does airway remodelling occur?
begins early on in the disease process of asthma and declining lung function, which is attributed to airway remodelling, is found in young children as well as new asthmatics
what changes may occur in the airway due to inflammation?
submucosal infiltration which activated lymphocytes and eosinophils, mast cell activation, epithelial changes and basement membrane thickening
how is airway modelling defined?
” an increase in goblet cells, hyperplasia and hypertrophy of smooth muscle cells leading to a thickened smooth muscle cell layar, increase airway deposition of collagen and other proteins, resulting in thickening of the lamina reticular with sub epithelial fibrosis and increased vascularity in the airway wall”
what factors influence airway remodelling?
TNF alpha, cysteinyl leukotrienes and eosinophils are major contributors
