Transient ischemic attack
a temporary episode of neurologic dysfunction (hypoxia that gets resolved)
Stroke
an acute neurological deficit that occurs when impaired blood flow to a localized area of the brain, resulting in injury to the brain
Ischemic stroke
blood supply to part of the brain is suddenly interrupted through a clot (embolic or thrombotic)
Management of ischemic stroke
tPA, heparin infusion, mechanical thrombectomy
Time frame for tPA
3-4 hours
Hemorrhagic stroke
bleeding into brain tissue/cranial vault
Hemorrhagic stroke management
coiling/stenting/clipping of aneurysm, craniotomy, burr holes, reverse clotting factors, HOB 30 degrees, osmotic fluids, treat BP
Penumbra
area of decreased perfusion but cells may recover
Subarachnoid hemorrhage
blood in space between the brain and arachnoid membrane
s/s of subarachnoid hemorrhage
horrible headache, nuchal rigidity
Treatment of subarachnoid hemorrhage
aneurysm repair, calcium channel blockers to prevent vasospasm
Right hemisphere stroke s/s
Left side changes, memory deficit, changes in thought process
Left hemisphere stroke s/s
Right side changes, language impairment
