What is cerebral palsy
a range of non-specific clinical symptoms characterized by abnormal motor patterns and postures
Causes of cerebral palsy
idiopathic, prenatal, perinatal, postnatal
Prenatal causes
congenital malformation, genetic abnormalities, hypoxia, preeclampsia, prematurity, maternal fever/seizure/bleeding
Perinatal causes
hypoxia, asphyxiation, placental complications, sepsis, CNS infection
Postnatal causes
head trauma, seizures, meningitis, intraventricular hemorrhage
Pathophysiology of CP
neurologic lesion resulting from abnormal development/damage in the motor areas of the brain
What type of disease is CP
nonprogressive disease - lesion doesn’t change
Spastic
hypertonicity, permanent contractures
Dyskinetic
abnormal, involuntary movements
What type of tone is the most likely to need surgery and orthotics
spastic
Ataxic
effects balance and depth perception
Goal of therapeutic management
assist child to gain optimal development and function within the limits of the disease
Health history
gestational/prenatal events, muscle weakness/rigidity, delayed developmental milestones
Physical exam
neuro/motor assessment, abnormal postures/range of motion, delayed developmental milestones
Laboratory/diagnostic tests
MRI/CT, cranial xrays, EEG, metabolic and genetic screening
