When does ARDs occur
within 72 hours of previously healthy children (sepsis, pneumonia, smoke inhalation, near drowning)
Pathology
alveolar-capillary membrane becomes more permeable –> pulmonary edema –> lung stiffness –> mucosal swelling and cellular debris –> atelectasis –> impaired gas diffusion
Goal of treatment
improve oxygenation and ventilation
What can it result in
residual lung disease, respiratory failure, death
What are the 1st symptoms
tachypnea, tachycardia
Why are high frequency ventilators used
to increase respiratory rate and low tidal volume
What does nitric oxide do
increase blood to the alveoli
What does liquid ventilation do
act as surfactant to improve gas exchange and increase pulmonary function
What is monitored closely
respiratory and cardiovascular status
Supportive cares
positioning, hygiene, pain/anxiety management
