Hypoplastic L heart syndrome
L side of the heart is underdeveloped, diminishing the ability for systemic circulation
Perfusion changes
blood from lungs going to the L atrium is obstructed, so blood flows through the ASD to the right side of the heart –> R ventricle pumps blood to the lungs and systemic circulation through the PDA
VS changes
Increased HR/RR, hypothermia
Treatment to keep ductus arteriosus open
administer prostaglandin
Primary treatment
heart transplant
Palliative 3 stage treatment
Norwood procedure, bidirectional glenn procedure, modified fontan procedure
Norwood procedure
reconstruction of aorta through small tube allowing blood flow to the lungs
When is the Norwood procedure done
right away
Bidirectional glenn procedure
increases blood flow to the lungs by connecting the superior vena cava to the R pulmonary artery
When is the bidirectional glenn procedure done
6 months
Modified fontan procedure
conduit placement, connect inferior vena cava to pulmonary artery for blood flow to the lungs
