What is tetanus?
Tetanus is an acute disease caused by neurotoxins from the bacterium Clostridioides tetani
Where are C. tetani spores usually found?
In contaminated soil
How does C. tetani usually enter the body?
They enter from contaminated soil via broken skin
What is the incubation period of tetanus?
4 days - several weeks
How does tetanus cause muscle spasm?
The toxin binds to inhibitor neurons, preventing release of inhibitory neurotransmitters
This leads to widespread activation of motor neurones and spasming of muscles throughout the body
How does tetanus usually present?
- Trismus (Lock jaw)
- Risus sardonicus (Abnormal sustained facial spasm and grinning)
- Autonomic instability

Abnormal sustained facial spasm and grinning
How will C. tetani test?
Anaerobic
Gram positive
Terminal spores
How can tetanus be avoided?
Routine vaccination
Wound management
What are some treatment options in tetanus?
- Surgical debridement
- Anti-toxins
- Booster vaccination
- Supportive measures (benzodiazapines, ß-blockers)
- Antibiotics (7-10 days)
