Extensor mechanism rupture Flashcards
What structures make up the extensor mechanism of the knee?
Tibial tuberosity
Patellar tendon
Patella
Quadriceps tendon
Quadriceps muscles
Who is most likely to develop an extensor mechanism rupture?
Middle aged men
Those who play running or jumping sports
Previous tendonitis
Steroid use
Chronic renal failure
Ciprofloxacin use
What are some symptoms of extensor mechanism rupture?
Knee pain
Weakness
What are some clinical signs of extensor mechanism rupture?
Lack straight leg raise
Palpable gap above knee
Reduced extensor power
What are some tests used in diagnosis of extensor mechanism rupture?
X-ray
USS
MRI
What will X-ray show in extensor mechanism rupture?
Effusion
Displaced patella
In which direction will the patellar be displaced in patellar tendon rupture?
Superiorly
In which direction will the patellar be displaced in quadriceps tendon rupture?
Inferiorly
What may be shown on USS or MRI in extensor mechanism rupture?
Slight or partial tears
How will extensor mechanism rupture be treated?
This requires urgent surgical repair with follow-up physiotherapy to gradually increase range of movement
How will small partial tears to the extensor tendons be treated?
Immobilisation and physiotherapy
Why are steroid injections not used in extensor mechanism tears?
They increase risk of rupture
