What is hallux valgus?
This is a condition in which there is a deformity of the great toe due to medial deviation of the 1st metatarsal and lateral deviation of the toe itself
What is the more common name for hallux valgus?
A bunion
Who is most likely to develop hallux valgus?
Females (3:1)
Those with a family history
What are some conditions that can predispose to hallux valgus?
- Ligamentous laxity
- Pesplanus
- Rheumatoid arthritis
- cerebral palsy
- 2nd toe amputation
What lifestyle choice can predispose to hallux valgus?
High heel usage
Narrow toe box shoe usage
Describe the pathophysiology of hallux valgus?
Valgus deviation of the phalanx =>
Varus deviation of the metatarsal head =>
Sesamoid region moves laterally as it remains attaches to the flexor hallucinations brevis
How will hallux valgus present physically?
Observable deformity
Joint pain
Transfer metatarsalgia
Fall risk and balance issues
How may hallux valgus present socially?
Patients may experience cosmesis, in which the person is bothered by the appearance
What are some examples of conservative management in hallux valgus?
- Analgesia
- Reassurance
- Low heel, wide shoes
- Physiotherapy (if tight gastrocnemius)
- Orthotics/Splints/Toe-spacers
When may operative management be used in hallux valgus?
In more severe cases, in which there is lifestyle limitations, failures of conservative management or lesser toe deformities
What does surgical management of hallux valgus involve?
Many osteotomies with the aim of realigning the hallux and decreasing the HV angle
