What is the recommended therapy for a pregnant woman with syphyllis?
single IM dose of Benzathine Penicillin G
What is the DOC for group A strep pharyngitis?
Penicillin V
What is another drug besides Penicillin V that can be used for treatment of GAS pharyngitis in young children?
Amoxicillin
For a patient who is unlikely to complete a full 10 day course of oral therapy for GAS pharyngitis, what is an alternative treatment?
Intramuscular benzathine penicillin G treatment
What may be used in patients with a type I hypersensitivity to penicillin’s?
- Azithromycin
- Clarithromycin
- Clindamycin
What may be considered in a patient with a non-type I allergy to penicillin?
1st gen cephalosporin (cephalexin or cepadroxil)
What is the treatment for congenital syphillis?
- Aqueous penicillin G IV (10 days)
- Procaine penicillin G IM (10 days) - alternative
How does the PK of penicillin G benzathine differ from aqueous crystalline penicillin G?
When given IM, it is slowly released and hydrolyzed to Penicillin G. Result: lower plasma levels but much more prolonged.
What is the first line therapy for uncomplicated UTI?
Co-trimoxazole
What is used for complicated UTI?
Ciprofloxacin
Why should Ceftriaxone be withheld in a pregnant woman with a UTI who is close to parturition?
Possibility of neonatal kernicterus
Nitrofurantoin
- indication: UTI in pregnancy
- A.E: Hemolytic anemia in G6PD deficiency
- Contraindicated at term
Sulfanomide pregnancy UTI contraindications
- Near term
- breastfeeding
- < 2 months (except as adjunctive therapy with pyrimethamine to treat congenital toxo)
A.E. of sulfanomides when contraindicated for UTI’s during pregnancy
Increase blood levels of unconjugated bilirubin and increase risk for kernicterus
DOC for Gonorrhea
Ceftriaxone plus doxycycline or azithromycin
