Pancreatic Enzyme Replacement
pancrelipase
What do you need to know for pancrelipase?
For CHRONIC pancreatitis only and you should take it with every meal and snack
Hyperosmotic laxative
lactulose
lactulose: MOA
Reduces blood ammonia absorption in hepatic encephalopathy
lactulose: Indications
-Reduction of ammonia absorption in hepatic encephalopathy
lactulose: NSG consideration
-PO or Enema
-Can be given to titrate by number of stools or by ammonia levels
-Not just given for high ammonia levels — must have S/S of encephalopathy
-Make sure patient is NOT hypokalemic
Do not give lactulose to patient who
just has high ammonia levels or is hypokalemic
rifaximin: MOA
-Inhibits bacterial RNA synthesis by binding to bacterial DNA (initially used as an antibiotic for GI infections)
rifaximin: Indication
Hepatic encephalopathy
rifaximin: Adverse Reactions
-Peripheral edema
-Nausea
-Ascites
-Dizziness
-Fatigue
-Pruritis
-Skin rash
-Abdominal pain
-Anemia
rifaximin: NSG considerations
-Associated with increased risk of C diff
ciprofloxacin: Indications
Used to treat UTI’s - STI’s and Anthrax
ciprofloxacin: adverse effects
Arthropathy (joint disease) Irreversible
Do not give to patient with bone pain
Do not give to patient under 12
ciprofloxacin: NSG consideration
Can have a prolonged effect and be concentrated in the neutrophils
Do not give to people with muscle and bone pain
Bactrim
sulfamethoxale and trimethoprim
