Cholelithiasis: Etiology
Impaired metabolism of cholesterol, bilirubin, and bile acids
3 types of cholelithiasis stones
Cholesterol (70%), pigmented, and mixed
Choleithiasis: Risk Factors
-Obesity
-Female
-Middle class
-Oral contraception
-Rapid weight loss
-Indigenous Americans
-Genetic predisposition
-Pancreatic or ileal disease
Fare-Fat-40
Choleithiasis Manifestations
-Usually asymptomatic
-Epigastric and right quadrant pain
-Intolerance to fatty foods and cabbage
-Billary colic
-Jaundice
Choleithiasis: Vague Symptoms
Heartburn
Flatulence
Epigastric discomfort
Food intolerances (fats and cabbage)
Biliary colic
Happens in choleithiasis and is very common to occur in 30 minutes to several hours after eating fatty meal
More severe cholelithasis
Jaundice from the stone blocking the bile duct
Severity of choleithisais depends on
Movement of stones
Obstruction
Clinical manifestation: Jaundice
Bile cannot flow into duodenum
Clinical manifestation: Dark amber urine and foams
Bilirubin backing up and getting into urine
Clinical manifestation: Clay colored stools
Bilirubin does not reach small intestine to be converted into urobilinogen
Clinical manifestation: Steatorrhea
No bile salts in duodenum, preventing fat digestion
CM: Pruritus
Deposits of bile salts into skin tissue
CM: Intolerance of fatty foods
No bile in small intestine to help with fat digestion
