Systemic steroids & Autoimmune conditions Flashcards
fludrocortisone mimic?
aldosterone
has mineralocorticoid activity
mineralocorticoid activity is?
maintain balance of water and electrolytes
Glucocorticoid activity
anti-inflammatory effects
Define HPA axis suppression
systemic steroids can cause the adrenal gland to stop producing cortisol due to feedback inhibition
-reason why steroids need to be tapered off so adrenal gland can resume cortisol production
Define Cushing’s syndrome
the adrenal gland produces too much cortisol or when exogenous steroids are taken in high doses and amount of cortisol produced is higher than what would have been produced naturally
Deffine Addison’s disease
adrenal gland is not making enough cortisol
S/Sx of cushing’s
psych changes achne GI bleeding DM hirsutism poor bone health infections growth retardation striae moon face
Steroids least potent to most potent
"Cute Hot Pharmacists and Physicians Marry Together & Deliver Babies" Cortisone Hydrocortisone Prednisone Prednisolone Methylprednisolone Triamcinolone Dexamethasone Betamethasone
What dose of steroids cause immunosuppression
> 2mg/kg/day or >20mg/day of prednisone
how to identify RA versus OA
bilateral, symmetrical disease where OA presents unilaterally
S/Sx of RA
joint swelling
pain-worse after rest
stiffness-worse after rest
bone deformity
preferred initial tx for RA
mtx
treatment for RA
DMARD
steroids
mtx 1st line
anti-TNF are add on therapy
list non-biologic traditional DMARDS
mtx hydroxychloroquine sulfasalazine leflunomide tofacitinib baricitnib
List anti-TNF biologic DMARDs
