granuloma = nodule full of macrophages
textbook presentation of sarcoid
young black female with dry cough, SOB + nodules on shins

majority spontaneously resolve in 6 months so if mild do nothing
otherwise: oral steroids + bisphosphonates (protect), methotrexate
