define conjunctivitis
inflammation of the tissue covering inside of eyelids + of the sclera
presentation of conjunctivitis
unilateral or bilateral
red eyes, bloodshot, itchy, gritty, discharging eyes
should be NO pain, photophobia or vision change
bacterial vs viral conjunctivits
bacterial - purulent discharge, very contagious, eyes may stick together in morning
viral - clear discharge + normal viral URTI symptoms, preauricular LNs, contagious
DDx painless red eye x 3
conjunctivitis
episcleritis
subconjunctival haemorrhage
DDx painful red eye x 7
glaucoma anterior uveitis scleritis corneal abrasions/ulceration keratitis foreign body traumatic,chemical injury
treating conjunctivitis
2 weeks usually self-resolves
good hygiene key for contacts
clean eyes with cooled boiled water and cotton wool
can give chloramphenicol eye drops or fusidic acid eye drops
why be careful with under 1 month olds + conjunctivitis?
need urgent ophthalmology review as may get loss of sight or pneumonia
allergic conjunctivit s
swelling of eye lid and watery discharge, itch
antihist, avoid allergen

acute anterior uveitis assoc
HLA B27 conditions
reactive arth
ank spond
IBD
chronic anterior uveitis assoc x4
sarcoid
syphilis
TB
herpes virus
presentation of anterior uveitis
unilateral+spontaneous with no obvious trigger (but may see presence of assoc condition, esp reactive arth)
dull achy red eye ciliary flush (red ring spreading from cornea outwards) floaters, flashes miosis photophobia excess tears abnormal iris shape
management of anterior uveitis
ophthalmology review
steroids
cyclopentolate, atropine (antimusc dilate pupil and stop ciliary spasm)
immunosuppressants
laser etc if v severe
