A
hot red swollen joint
stiff + poor ROM
fever, tiredness, sepsis

most common cause of septic arth
A
staph aureus

single acutely swollen joint
urinary + genital symptoms
A
always exclude gonococcal arthritis first!!!
then reactive arthritis next Dx (remember can be triggered by gastroenteritis too, conjunctivitis also seen)

DDx x 4 for septic arth
A
gout - urate -ve biref pseudogout - Cpyro +ve bi reactive arth reactive arthritis haemarthrosis

management process for ? septic joint
A
low threshold
follow hot joint policy
send sample for gram stain, crystal microscopy, culture, sensitivities
fluclox + rifampicin may be first line
3 to 6 weeks IV
